A Better and More Prosperous New Hyde Park

The UNDERSIGNED hereby subscribes and promises to pay $125.00 annually for a business membership or $25.00 annually for a personal membership to The Greater New Hyde Park Chamber of Commerce, Inc., as an investment in a Better and More Prosperous New Hyde Park. This agreement replaces all agreements of prior date and is subject to approval of the Board of Directors. This membership, payable in advance, shall continue from year to year unless cancelled by written notice one month prior to the close of any annual period. The effective date of membership shall be the date your check in the amount of $125.00 for business or $25.00 for personal is received at the address listed above.
